Top Things That Can Disqualify You During a DOT Exam
If you’re a commercial driver preparing for your DOT physical exam, it’s important to know what could lead to disqualification. The FMCSA (Federal Motor Carrier Safety Administration) has clear medical standards, and failing to meet them can mean you won’t receive your DOT medical certificate—or worse, you could lose your commercial driving privileges.
At DOT Modesto, we’re here to help you pass with confidence. Below are the most common things that can disqualify you during a DOT exam so you can prepare ahead of time.
1. Uncontrolled High Blood Pressure
If your blood pressure is too high (usually over 180/110) and not being treated, you could be disqualified. Controlled high blood pressure is acceptable, but you may need more frequent checkups.
✅ Pro Tip: Take your meds, avoid salty foods, and skip energy drinks before your exam.
2. Poor Vision That Can’t Be Corrected
Drivers must have at least 20/40 vision in each eye (with or without glasses/contacts) and a field of vision of at least 70° in the horizontal meridian in each eye.
❌ If you can’t meet this standard—even with corrective lenses—you may be disqualified.
3. Hearing Loss
You must be able to hear a forced whisper from 5 feet away or pass an audiometric test.
✅ Hearing aids are allowed if they help you meet the requirement.
4. Diabetes Not Under Control
Drivers who use insulin are now allowed to drive if they have proper documentation from their healthcare provider and maintain blood sugar control.
❌ Uncontrolled diabetes or lack of required paperwork may disqualify you.
5. Use of Certain Medications
Taking medications like narcotics, benzodiazepines, or certain anti-seizure drugs may disqualify you—especially if they affect alertness or reaction time.
✅ Always bring a letter from your doctor if you’re on a prescribed medication that could raise concerns.
6. Epilepsy or Seizure History
Drivers with active epilepsy or recent seizures may be disqualified. Some drivers may qualify with special exemptions if they’ve been seizure-free for a certain period.
7. Cardiovascular Disease
Conditions like recent heart attacks, uncontrolled arrhythmias, or heart failure can disqualify you unless you’re cleared by a cardiologist.
✅ Bring all recent cardiac test results and clearance letters to the exam.
8. Drug or Alcohol Abuse
A history of alcohol or drug abuse—especially with a failed DOT drug/alcohol test—can lead to immediate disqualification unless the driver has completed a treatment program and has proper documentation.
9. Sleep Apnea (Untreated or Non-Compliant)
Drivers with moderate to severe sleep apnea must show they are using a CPAP machine regularly and have sleep logs from the past 30–90 days.
10. Missing Medical Records
If you’re under treatment for a serious condition and don’t bring documentation (such as clearance letters or test results), the examiner may withhold certification until you provide them.
The Bottom Line
Most of these conditions don’t automatically disqualify you—as long as they are well-managed and you have the proper documentation.
