If you’re a commercial driver or about to become one, you’re probably wondering what to expect from a DOT physical exam. The process may sound intimidating, but it’s actually pretty simple—and we’ll walk you through it.

At DOT Modesto, we make DOT exams fast, professional, and stress-free. Here’s a step-by-step look at what happens during a DOT physical so you can show up confident and prepared.

What Is a DOT Physical?

A DOT physical is a medical exam required by the Federal Motor Carrier Safety Administration (FMCSA) to make sure you’re healthy enough to safely operate a commercial motor vehicle (CMV).

It must be completed by a FMCSA-certified medical examiner—like the ones at DOT Modesto.

What Happens During the Appointment?

Here’s what your exam will typically include:

1. Medical History Review

We’ll start by asking you questions about:

  • Past or current medical conditions
  • Surgeries or hospitalizations
  • Medications you’re taking
  • Any use of tobacco, alcohol, or substances
  • Sleep habits and fatigue
  • Seizure or neurological history

Tip: Be honest—it helps us provide the best care and prevent delays.

2. Vision Test

You’ll need 20/40 vision or better in both eyes, with or without glasses or contacts.

We’ll check:

  • Visual acuity
  • Peripheral vision
  • Color recognition (especially red, green, and amber)

3. Hearing Test

You must be able to:

  • Hear a forced whisper from at least 5 feet away, OR
  • Pass an audiometric test (hearing test in decibels)

Hearing aids are allowed if they help you meet the standard.

4. Blood Pressure & Pulse Check

High blood pressure can impact your driving safety.
We’ll:

  • Take your blood pressure
  • Check your pulse rate and rhythm

If you have a history of hypertension, we may need documentation that it’s under control.

5. Urine Test

This is not a drug test (unless requested by your employer).
It checks for:

  • Blood sugar (diabetes screening)
  • Protein (possible kidney issues)
  • Other health markers

Be sure to drink water before your appointment so you’re ready for the sample.

6. Physical Examination

We’ll do a full-body checkup, including:

  • Heart and lung sounds
  • Neurological reflexes
  • Spine and joint mobility
  • Abdomen, skin, and general appearance
  • Listening for signs of sleep apnea

The goal is to catch anything that could impair your ability to drive safely.

7. Certification Decision

If you meet all the medical standards, you’ll receive:

  • A DOT Medical Examiner’s Certificate (MEC)
  • A Medical Card valid for up to 2 years

If you have a condition that needs monitoring, you may be approved for a shorter period (like 3, 6, or 12 months).

What If I Don’t Pass?

Don’t panic. Sometimes, you just need to:

  • Provide more documentation
  • See your specialist
  • Adjust medications
  • Reschedule once you’re cleared

We’re here to help you get back on the road safely.
