If you’re a commercial driver, you already know that passing your DOT physical exam is essential for keeping your CDL and staying on the road. But if you’re new to the process—or even if you’ve done it before—you probably have questions.

1. What Is a DOT Physical, and Who Needs It?

A DOT physical is a required medical exam for commercial drivers who operate:

  • Vehicles over 10,000 lbs
  • Passenger vans/buses carrying 8+ people for pay or 15+ for free
  • Any vehicle transporting hazardous materials

The exam checks your health to make sure you’re safe to drive. It must be done by a FMCSA-certified medical examiner.


2. How Long Does a DOT Physical Take?

Most appointments take about 20 to 30 minutes, depending on your health history and how prepared you are.

Tip: Bring your ID, glasses, medication list, and any specialist paperwork if needed.


3. What Do I Need to Bring With Me?

  • A valid driver’s license or ID
  • Eyeglasses/contacts (if you wear them)
  • List of medications and conditions
  • Medical documents (if you have issues like diabetes, heart disease, or sleep apnea)
  • CPAP report (if applicable)
  • Your current DOT medical card if renewing

4. What Gets Checked During the Exam?

You’ll go through:

  • Medical history review
  • Vision and hearing test
  • Blood pressure and pulse check
  • Urine test (not a drug test)
  • Physical exam (lungs, heart, reflexes, etc.)

5. Is There a Drug Test?

No—a DOT physical does not include a drug test unless your employer specifically requests one. However, random or pre-employment drug testing is required separately by the DOT.


6. How Long Is the Medical Card Good For?

If you pass, your DOT Medical Examiner’s Certificate is good for up to 2 years.
If you have certain health conditions, it may be valid for 3 months, 6 months, or 1 year.


7. What If I Have a Medical Condition?

It depends on the condition. Many drivers can still qualify with proper documentation and control of their condition (e.g. high blood pressure, diabetes, or sleep apnea).

  • Recent lab results
  • CPAP usage logs

8. What If I Fail the Exam?

You may not “fail” permanently. Often, you’re just temporarily disqualified until:

  • You bring required documents
  • See your specialist
  • Adjust your treatment or medications

Yes! You can go to any FMCSA-certified medical examiner listed on the National Registry.
