You do not turn this in yourself—your examiner keeps it for records.


2. Medical Examiner’s Certificate (MEC or “DOT Card”) – MCSA-5876

This is your proof that you passed your exam. It’s a small card that includes:

  • The date of your exam
  • The expiration date (usually 1–2 years)
  • Your examiner’s signature

This must be kept with you at all times while driving.


3. Insulin-Treated Diabetes Mellitus Assessment Form

If you take insulin for diabetes, the FMCSA requires this form from your treating provider to confirm that your condition is well-managed and safe for driving.


4. Vision or Hearing Exemption Forms

If you have vision or hearing loss and were previously approved for an FMCSA exemption, you’ll need to renew and submit these forms with your exam.


5. Medication Use Documentation

Not an official FMCSA form, but if you’re on certain medications (e.g., opioids, stimulants, or seizure meds), you may need a letter from your doctor explaining:

  • Why the medication is necessary
  • That you are stable and safe to drive
  • That the meds won’t impair your driving ability
