What to Bring to Your DOT Appointment
Getting a DOT physical exam is an important step in keeping your CDL active and staying safe on the road. To make your visit fast and smooth, it’s important to show up fully prepared.
At DOT Modesto, we help commercial drivers complete their exams with confidence. Here’s a quick checklist of exactly what to bring to your DOT appointment—so there are no surprises.
✅ 1. Valid Driver’s License or ID
Bring your current driver’s license or any valid government-issued photo ID. This helps confirm your identity and ensures proper reporting of your results.
✅ 2. Eyeglasses or Contact Lenses
If you wear corrective lenses, bring them to your exam. Your vision must meet 20/40 in each eye (with or without correction) to pass.
✅ Bonus Tip: If you have backup glasses or a case for your contacts, bring those too.
✅ 3. List of Medications
Create a written list of all prescription and over-the-counter medications you take. Be sure to include:
- Name of the medication
- Dosage and frequency
- Why you’re taking it
- Name of the prescribing doctor
✅ 4. Medical Records for Ongoing Conditions
If you’re being treated for anything like:
- High blood pressure
- Diabetes
- Sleep apnea
- Heart disease
- Seizures or neurological disorders
…you’ll need to bring supporting medical records or doctor’s clearance letters.
✅ 5. CPAP Compliance Report (if you have sleep apnea)
Drivers with sleep apnea must bring a report showing at least 30 days of CPAP usage (usually downloaded from your machine or app). Without it, you may not pass.
✅ 6. Hearing Aids (if applicable)
If you use hearing aids, bring them with you—plus extra batteries just in case. We’ll test your hearing as part of the exam.
✅ 7. Your Current DOT Medical Card (if renewing)
If this is a renewal exam, bring your previous DOT card with you so we can update your record properly.
✅ 8. Payment
Bring your preferred payment method—cash, card, or other (check your clinic’s accepted options in advance). At DOT Modesto, we offer affordable pricing and fast service.
✅ 9. A Full Bladder
You’ll need to complete a urine test as part of the exam (not a drug test, but for checking protein, sugar, etc.). Arrive well hydrated so you’re ready to go.
Optional (but helpful):
- Copies of your blood pressure readings if you’re borderline
- A list of surgeries or hospitalizations in the past 5 years
- Your blood sugar logs if diabetic
