✅ Can You Be DOT Certified with Diabetes?
Yes. The FMCSA allows commercial drivers with both Type 1 and Type 2 diabetes to be certified—as long as the condition is well-managed and doesn’t interfere with safe driving.
🩺 Two Categories of Diabetic Drivers
1. Drivers Who Do NOT Use Insulin
- If your diabetes is controlled with diet, exercise, or oral medications, you can be certified for up to 1 or 2 years.
- You’ll still need to pass the urinalysis and physical exam.
2. Drivers Who Use Insulin
- You can still be certified, but you must bring:
- Your Insulin-Treated Diabetes Mellitus Assessment Form (MCSA-5870)
- A signed letter from your treating clinician (like an endocrinologist or PCP)
- Proof of stable insulin use and no recent hypoglycemic events
📄 What Is the MCSA-5870 Form?
This is a special document your doctor fills out to confirm that:
- You know how to manage your insulin
- Your blood sugar levels are well controlled
- You haven’t had any severe low blood sugar episodes in the past 12 months
You must bring this form to your DOT exam—or the examiner cannot certify you.

- Uncontrolled diabetes with frequent high or low blood sugar
- Recent episodes of hypoglycemia (low blood sugar that caused confusion, seizure, or loss of consciousness)
