How Sleep Apnea Impacts DOT Physical Eligibility
Sleep apnea is a common but serious condition that affects breathing during sleep—and it can impact your ability to pass a DOT physical and keep your CDL.
At DOT Modesto, part of Affection Health Care, we’re here to help you understand how FMCSA views sleep apnea, what drivers need to know, and how you can stay compliant and safe on the road.
😴 What Is Sleep Apnea?
Sleep apnea is a condition where breathing repeatedly stops and starts during sleep. The most common form is obstructive sleep apnea (OSA).
It can cause:
- Loud snoring
- Daytime fatigue
- Difficulty concentrating
- Increased risk of accidents
Because of these safety concerns, the FMCSA requires commercial drivers to be screened for signs of sleep apnea during their DOT exam.
❓ How Does Sleep Apnea Affect My DOT Certification?
If you show signs of moderate to severe sleep apnea that’s not treated, you may not be certified to drive. That’s because untreated sleep apnea can:
- Cause you to fall asleep while driving
- Increase your risk of crashes
- Affect your alertness and reaction time
✅ You Can Be Certified If Your Sleep Apnea Is Treated
The FMCSA allows drivers with sleep apnea to be certified if:
- You’re being treated (usually with a CPAP machine)
- You’re using the CPAP at least 4 hours per night, 70% of nights
- You provide proof of compliance (usually a CPAP usage report)
If your treatment is working and you’re following the plan, you can be DOT certified—often for a 1-year card.
🔍 Signs That May Trigger a Sleep Apnea Evaluation
During your exam, the medical examiner may ask more questions if you:
- Have a BMI over 35
- Have a large neck circumference
- Snore heavily
- Report daytime sleepiness
- Have a history of high blood pressure or diabetes
If your answers raise concern, the examiner may request a sleep study before issuing your card.
📝 What to Bring If You’ve Been Diagnosed
If you’ve already been diagnosed with sleep apnea and are using treatment:
- Bring a letter from your sleep specialist
- Bring a CPAP compliance report from the past 30 days
- Bring a list of medications or treatments, if any
⚠️ What If You Refuse Treatment or Evaluation?
If the examiner believes you may be at risk and you refuse a sleep study or treatment, you can be disqualified until you follow through.
💤 Tips to Prepare for Your DOT Exam
- Get a good night’s sleep before your exam
- Be honest about your symptoms and medical history
- Bring CPAP documentation if applicable
