What Medications Can Disqualify You from a DOT Card?

If you’re getting ready for a DOT physical, you might be wondering if any of your current medications could prevent you from passing the exam.

At DOT Modesto, part of Affection Health Care, we help commercial drivers understand the FMCSA medication guidelines—so you can be prepared and avoid surprises.

💊 Why Medications Matter in a DOT Physical

Certain medications can affect your:

  • Alertness
  • Reaction time
  • Judgment
  • Ability to operate a commercial vehicle safely

That’s why the Federal Motor Carrier Safety Administration (FMCSA) has rules about which medications are allowed—and which may lead to temporary or permanent disqualification.

🚫 Medications That Can Disqualify You

You may be disqualified if you are currently taking the following types of medications without proper clearance:

🔹 Controlled Substances (Schedule I)

  • Heroin
  • LSD
  • Ecstasy
  • Marijuana (even if legal in your state)

These are always disqualifying under federal law—even with a prescription or medical marijuana card.

🔹 Narcotic Pain Medications

  • Oxycodone (OxyContin)
  • Hydrocodone (Norco, Vicodin)
  • Morphine
  • Fentanyl

Drivers using these medications regularly may be disqualified unless:

  • The medication is prescribed by a licensed medical provider
  • The prescribing provider writes a letter stating that it will not impair driving
  • The driver shows no signs of side effects or misuse

🔹 Anti-Seizure Medications

Used to treat epilepsy or other seizure disorders. If you are taking these due to a seizure history, you may be required to:

  • Be seizure-free for a minimum of 8–10 years without meds
  • Provide specialist documentation

🔹 Psychotropic Medications

  • Benzodiazepines (e.g., Xanax, Valium, Ativan)
  • Antipsychotics (e.g., Seroquel, Risperdal, Abilify)
  • Some antidepressants (especially sedating types)

These can be disqualifying if they impair your ability to drive safely. A case-by-case evaluation will be needed.

🟡 Medications That Require Further Evaluation

These aren’t automatic disqualifiers, but they do raise concerns:

  • Sleep aids (e.g., Ambien)
  • Muscle relaxers
  • ADHD medications (e.g., Adderall, Ritalin)
  • Methadone or Suboxone (for substance use disorder)

You’ll need a written statement from your prescribing doctor confirming:

  • The medication does not impair your alertness
  • You’re stable on the medication
  • You’re safe to drive

📝 What to Bring to Your DOT Physical

If you take any medications:

  • Bring a list of all current prescriptions and dosages
  • Include the reason for each medication
  • Bring a letter from your prescribing doctor if the medication affects the nervous system

Being upfront helps avoid delays in your certification.
